The Nutritional Immunology and Molecular Medicine Laboratory (NIMML) research team at Virginia Tech has discovered important new information on the efficacy of conjugated linoleic acid (CLA) in treating Crohn's disease, a form of inflammatory bowel disease (IBD). CLA is a naturally occurring acid found in meat and dairy products known for its anti-cancer and immune modulatory properties.
In collaboration with the Division of Gastroenterology and Hepathology at University of North Carolina School of Medicine and the Wake Forest Medical Center, researchers found that Crohn's patients who took supplementary CLA showed noticeable improvement. "In our recent open label study of CLA as a supplement in study subjects with mild to moderate CD there was a marked improvement in disease activity and quality of life in 50% of the subjects. CLA was well tolerated by all of the study subjects. These findings are very encouraging and will need to be verified in a randomized controlled trial," said Professor Kim L. Isaacs, a Professor of Gastroenterology at the University of North Carolina at Chapel Hill.
The two main manifestations of IBD?Crohn's and ulcerative colitis?afflict over 1.4 million people in the United States. Symptoms include abdominal cramping, fever, fatigue, loss of appetite, skin and mouth ulcers, and diarrhea or constipation. In addition, the risk of developing colorectal cancer increases by about one percent yearly in IBD patients. Currently, there is no cure for Crohn's disease and the exact causes of it aren't fully understood.
CLA affords those afflicted with mild to moderate IBD an effective treatment without the unwanted side effects of many synthetic drugs. "Furthermore, we have demonstrated that probiotic bacteria can produce CLA locally and suppress colitis. Therefore, CLA can be administered directly in capsules or indirectly through CLA-producing probiotic bacteria," said Dr. Raquel Hontecillas, an Assistant Professor of Immunology at NIMML.

"The validation of the anti-inflammatory actions of CLA in the gut is in line with our goal because CLA is a natural fatty acid found in milk and ruminant products. The fully integrated bioinformatics, nutrition and immunology experimentation capabilities of NIMML enable the acceleration of translational biomedical research from computational and mathematical modeling into the clinic. CLA is an example of an anti-inflammatory compound in a pipeline of naturally occurring and synthetic compounds (e.g., abscisic acid, eleostearic acid, terephthalanilides) with tremendous therapeutic and prophylactic potential as anti-inflammatories," said Dr. Josep Bassaganya-Riera, a Professor of Immunology, principal investigator of this human clinical trial, and the Director of the NIMML and the Center for Modeling Immunity to Enteric Pathogens.
These findings, reported in the most recent edition of Clinical Nutrition1, were awarded the American College of Gastroenterology Presidential Poster of distinction for human clinical trials.

OAK BROOK, Ill. New research shows that the addition of two magnetic resonance imaging (MRI) sequences to a common MR angiography technique significantly improves detection of pulmonary embolism, a potentially life-threatening condition traditionally diagnosed through computed tomography (CT). Results of the study are published online in the journal Radiology.
Pulmonary embolism occurs when a blood clotusually from the legtravels to the lung and blocks the pulmonary artery or one of its main branches. CT angiography is the gold standard for diagnosis, but it exposes patients to ionizing radiation and iodinated contrast agent, which carries a risk of allergic reactions and kidney damage in some patients.
"MRI is developing much faster than CT," said Diego R. Martin, M.D., Ph.D., head of the Department of Radiology at the University of Arizona College of Medicine in Tucson. "The images we're getting are already significantly better than they were a year ago. There is no doubt that in the future we will be able to offer a non-radiation-based alternative to CT for the diagnosis of pulmonary embolism."
MRI has been used for pulmonary embolism detection in pregnant women and patients whose kidneys may be harmed by CT angiography contrast agents. However, the Prospective Investigation of Pulmonary Embolism Diagnosis III (PIOPED III) study, a major multicenter trial, found that centers had difficulty obtaining adequate quality MR pulmonary angiography (MRPA) for suspected pulmonary embolism. The PIOPED researchers determined that MRPA should be considered only at centers that routinely perform it and perform it well, and for patients who have contraindications to standard tests.
In the new study, the research team assessed the impact of two additional MRI sequences on MRPA's accuracy. The two techniquescontrast-enhanced volumetric interpolated breath-hold examination (VIBE) and non-contrast true fast imaging with steady-state precession (true FISP)complement MRPA, according to Dr. Martin.
The addition of VIBE provides a gray scale that enables readers to distinguish between the clot, or thrombus, and the lung, which both appear dark on MRPA.
"Also, VIBE is not time sensitive," Dr. Martin said. "If the patient coughs, you can do it again. You don't have that option with MRPA."
The true FISP test does not require contrast agent or a breath hold, an important consideration for embolism patients who often cannot hold their breath long enough for image acquisition on MRPA.
When Dr. Martin and colleagues studied the three techniques on 22 patients with CTA diagnosis of pulmonary embolism, they found a sensitivity of 55 percent, 67 percent and 73 percent for MRPA, true FISP and VIBE, respectively. Combining all three MRI sequences improved the overall detection rate to 84 percent. Specificity was 100 percent for all detection methods except for MRPA, which demonstrated one false positive.
Dr. Martin hopes his findings stimulate discussion of a new, redesigned PIOPED study.
"PIOPED III did not answer the question of whether or not MRI is a useful alternative to CT, because it didn't use all the tools available," he said.
The MRI sequences used in the study are offered on all the major vendor systems, according to Dr. Martin, with the entire three-sequence protocol taking only 15 minutes to set up and perform.

We investigated whether and how individual differences in personality determine cognitive training outcomes. Forty-seven participants were either trained on a single or on a dual n-back task for a period of 4 weeks. Fifty-two additional participants did not receive any training and served as a no-contact control group. We assessed neuroticism and conscientiousness as personality traits as well as performance in near and far transfer measures. The results indicated a significant interaction of neuroticism and intervention in terms of training efficacy. Whereas dual n-back training was more effective for participants low in neuroticism, single n-back training was more effective for participants high in neuroticism. Conscientiousness was associated with high training scores in the single n-back and improvement in near transfer measures, but lower far transfer performance, suggesting that subjects scoring high in this trait developed task-specific skills preventing generalizing effects.?We conclude by proposing that individual?differences in personality should be considered in future cognitive intervention studies to optimize the Working memory training efficacy of training.
